CLASS Q ALLOTMENT (IM-15); FORMS MANUAL REVISION #2
DISCUSSION: 
The purpose of this memorandum is to inform staff that the Class Q Allotment (IM-15) form is now obsolete.  All remaining copies in the counties should be destroyed.
NECESSARY ACTION: 
  • Review this memorandum with appropriate staff.
  • Discontinue the use of the IM-15.
  • Make pen and ink changes to the Forms Manual to obsolete this form.
