DISCUSSION:

Form IM-69, Notification of Saved Food Stamp Benefits pdf logo and instructions are revised. The IM-69 is used to provide the EU with information regarding the aging process when the individual has requested to save his/her Food Stamp benefits. The changes are as follows:

The IM-69 PDF and WORD versions will be available on the FSD intranet. A printed form will not be available from the warehouse.

To access the PDF version and/or instructions,

To complete the WORD version,

Destroy/recycle all versions of the IM-69 with a revision date prior to 01/08.
