SUBJECT: | FORM IM-69, NOTIFICATION OF SAVED FOOD STAMP BENEFITS AND INSTRUCTIONS |
DISCUSSION:
Form IM-69, Notification of Saved Food Stamp Benefits and instructions are revised. The IM-69 is used to provide the EU with information regarding the aging process when the individual has requested to save his/her Food Stamp benefits. The changes are as follows:
- change Division of Family Services to Family Support Division;
- change caseworker to eligibility specialist;
- change the text to more clearly state the aging process; and
- instruct the EU to keep track of benefits to have them replaced.
The IM-69 PDF and WORD versions will be available on the FSD intranet. A printed form will not be available from the warehouse.
To access the PDF version and/or instructions,
- go to Family Support Division (FSD) Intranet site,
- select Food Stamps,
- under Forms select Forms Manual from the drop-down list,
- select Volume 2, and
- select PDF IM-69 or instructions.
To complete the WORD version,
- go to D:\FSFORMS and
- select the IM-69.
Destroy/recycle all versions of the IM-69 with a revision date prior to 01/08.
