SUBJECT:  REQUEST FOR PARTICULAR MISSING VERIFICATION – 2nd REQUEST FOR INFORMATION (FA-325) – FOOD STAMP PROGRAM

DISCUSSION:

The Food and Nutrition Service (FNS) provided guidance in regards to providing Food Stamp participants time to provide information that is requested by Family Support Division (FSD).

When an initial Request for Information (FA-325) is sent to the participant and only part of the required verification is provided, the Family Support Division (FSD) is required under Section 273.2(h)(1)(i)(C) to request the verification that is still missing. If partial verification is provided within the ten day period of the initial FA-325, a second FA-325 must be sent to the client, giving the household ten days to provide the specific missing verification.

The 2nd FA-325 must list in detail what information is needed from the client. The form must be manually generated and fields populated with specific detail as on what is still needed from the participant. The 2nd FA-325 can be printed in the overnight FAMIS batch printing.   • If partial verification is provided within the ten day period, a second Request for Information (FA-325) must be sent to the client.
  • If all verification is provided during the 10 day period of the 2nd FA-325 staff must ensure that the case is properly processed. If the case has already been closed by FAMIS, staff must complete the cancel/close action to ensure the verification is properly processed prior to case approval or rejection.

Example:

On November 9, 2017 Sam Smith submits a change report that states he had a change in work hours and that he received a wage increase. The same day the Eligibility Specialist (ES) sends Mr. Smith the FA-325 requesting specific verification of the new income (4 pay stubs, as he is paid weekly). The information is due 11/20/2017. On 11/15/2017 Mr. Smith provides one check stub showing the change in hours. Because Mr. Smith does not provide all requested pay stubs, the worker will then need to generate a 2nd FA-325 listing specifically what is needed from Mr. Smith (3 additional pay stubs). The 2nd FA-325 is due 10 days from the date the 2nd FA-325 is issued.
