IM-1SSL Signature Request Letter
DISCUSSION:
Clarification has been added to section 1802.020.05 Application with No Signature of the Family MO HealthNet (MAGI) manual. The name and address requirement to make an application valid has been removed. This section now states that:
- If address information is missing or an invalid mailing address is given, staff should attempt to locate an address through FAMIS, MODL, IIVE or Work Number. If staff are unable to establish a valid address, the application should be desk rejected for reason of “Unable to Locate.”
Staff should also return any original unsigned IM-1SSL application with the IM-1SSL Signature Request Letterand an FSD addressed envelope to the customer within three business days.
Clarification has also been added to section 1120.010.00 APPLICATIONS RECEIVED BY THE AGENCY of the Food Stamp manual. The section now states the following:
- An FS-1 missing the applicant’s signature is not considered an application.
- When an FS-1 is received without a signature return the original unsigned application with the Signature Request Letter, and an FSD addressed envelope to the customer within three business days.
The reference that the return envelope is stamped has been removed from the MAGI and Food Stamp manuals as well as the signature letters.
